As of July 1st 2017 we are required to have written consent to have your email in our system. We will be mailing out email consent forms with a return stamped envelope within the next few weeks, please sign the consent form, mail it back to the clinic and we will update your file. We will be removing emails from files that do not have written consent by July 1st 2017. If you do not receive a consent form in the mail please give us a call 705-560-7226, and we can send you another consent form or stop by the clinic to fill it out in the office. For clients with an account number 9500 and higher the consent was attached to your new client form so you will not receive a consent form in the mail.
